Health Risk Assessment
Each year as part of our vision, Taking You Well Into The Future, Methodist Medical Center offers it's employees and their spouses an opportunity to participate in an annual Health Risk Assessment.
These Health Risk Assessments are conducted annually beginning October 1st and concluding December 9th.
All employees and their spouses who utilize Methodist's insurance program (UMR) are offered the opportunity for a Wellness Incentive Discount for participation in a Health Risk Assessment. Both the employee and the spouse (if on the policy) must participate to receive the Wellness Incentive Discount.
Health Risk Assessments consist of 3 parts. All portions must be completed in there entirety by all insured parties in order to be eligible for the Wellness Incentive Discount. A fourth portion is recommended for participants who wish to view the summary of their On Line Health Risk Assessment.

Biometric Measurements

Though biometric testing the Health Risk Assessment is able to risk stratisfy participants. These risk stratifications are used to determine each participant's individualized Lifestyle Coaching profile.
Biometric measurements include the following factors and must be conducted by a participant's physician, LifeStyle Coach or through a scheduled Health Risk Assessment Screening.
All biometric measurements require an 8-12 hour fast prior to testing and should include:
- Blood Glucose
- Participants with a Blood Glucose over 126 will be required to participate in a Hemoglobin A1C, as well
- Blood Pressure
- Cholesterol
- High Density Lipoproteins
- Low Density Lipoproteins
- Triglycerides
- Ratio of High Density Lipoproteins to Total Cholesterol
- Height
- Weight

This 80 question assessment provides valuable insight into your personal wellness. Data from this questionnaire is compiled and sent to participants' MyMethodist E-Health Accounts.
LifeStyle Coaching
In February following the Health Risk Assessment each Methodist Employee and spouse will be risk stratified based upon their Biometric Measurements and assigned a LifeStyle Coach. LifeStyle Coaching models are determined by the risk stratification of their individual Biometric Measurements.
LifeStyle Coaching provides Methodist employees and their spouses with an opportunity to sit down with a specially trained LifeStyle Coach to establish goals for improving overall health, learn about various Wellness Programs Methodist has to offer and monitor Biometric Measurements throughout the year.

MyMethodist E-Health (This portion is recommended for participants who wish to view the summary of their On Line Health Risk Assessment.)
To provide participants a secure and confidential method for accessing the results of their Online Health Risk Assessment participants have the option of registering for My Methodist E-Health.
This service provides participants’ access to their Health Risk Assessment as well as resources to connect with Primary Care Physicians, receive lab results and track their own health records.
