An assessment includes an interview with the patient and family, a physical exam performed by a geriatrician (a doctor specializing in the problems of aging), social workers or other health care professionals. Once the assessment is complete, our team meets with the patient and family to discuss a treatment plan.

Family physicians, adult children, other family members, as well as the patients themselves can refer a patient to our clinic.
