Heart Failure Support Group

Living With Heart Failure is a heart failure support group for patients and their families.

Every month you'll learn more about how to better manage your heart failure.  A pharmacist, dietitian and nurse practitioner will be available to answer any questions you have.  Please feel free to bring a family member or guest with you.

Heart healthy refreshments will be served!
