After you attend our FREE information seminar...
- Call your insurance company to verify that your policy covers weight loss surgery, and ask what qualifications must be met for approval. Click here for information about payment and insurance.
- Call our office at 309-671-7764 or 866-599-NEW U to schedule your first appointment. You will see Dr. Salimath, and based on your information and health assessment, he will decide whether or not you are a candidate for weight loss surgery and which procedure would be best for you.
- If Dr. Salimath determines you are a candidate for surgery, you will be given prescriptions necessary to get all pre-surgery testing and assessments done. The following are REQUIRED for surgery and must be completed prior to scheduling your surgery:
- Consultation with our Bariatric Registered Dietitian and Exercise Specialist. The combined non-refundable $420 fee is your responsibility. Click here for details.
- Consultation with our Licensed Clinical Psychologist or a Psychiatrist.
- Diagnostic tests/labs.
- Depending upon your individual health situation, you may need to see other health care specialists.
- Lastly, your family physician will need to see you for a pre-surgery clearance.
- After all your tests/labs, consultations and payment verification are complete, we will set a surgery date with you. It usually takes about 3-6 months from your first appointment until your surgery date.
- You will visit our office 2 weeks before your surgery, then 2 weeks, 2 months, 6 months, and 1 year after your surgery. Our whole team will be checking in with you at each visit to make sure you are healthy and happy with your results.
